Application Checklist for Housing

Use this list to keep track of what you may need to submit to the housing authorities. You may need to submit similar documentation multiple times. As you collect it, be sure to:

• 1. 1. Check off which information you have
     2. Write the date you got the document
     3. Place the originals in an envelope in a safe place
     4. Make copies to mail to housing provider

• Copy of Birth Certificate(s) for all household members

• Copy of Social Security Card(s) for all household members

• Verification of Income

• • Social Security Benefit Letter from the last month
  • Earned Wages, like a W-2 Form, multiple pay stubs, letter from employer
  • Child Support
  • DTA Benefits
  • Other

• Verification for Priority Housing

• • Displaced by Natural Forces
    • Copy of Official Fire/Flood Report
    • Proof that you were resident of affected property
  • Displaced by Public Action
    • Official Condemnation Order
    • Proof that you were resident of affected property
  • Homeless
    • Letter from Homeless Shelter or person you’re staying with
    • Proof that you were evicted for no-fault reason, including remodeling, condo conversion, landlord moves in with own family, etc.
    • Proof that you were resident of affected property
  • Abuse
    • Police reports or court documents
    • Letter from counselor or social service agency
    • Letter from attorney
    • Proof that you were displaced from your primary residence
  • Severe Medical Emergency
    • Letter from your medical provider
    • Proof that your current primary residence is unsafe due to your medical condition
    • Proof that you were displaced or at-risk of being displaced

• Verification of Assets

• • 3 most recent banking statements for all accounts

• Verification of Disability
