Options Counseling

Options CounselingOptions Counseling can help when an elder or an adult of any age with a disability faces challenges and must make tough choices. The person may be faced with losing their home or needing to decide if they can remain in their home. They may need support services to help with basic activities such as personal care, household chores, transportation, and nutrition or medication management.  Locating and evaluating options can be a difficult task. The choices are varied and can feel overwhelming:

The decision can be made more manageable with the support of a knowledgeable Options Counselor who will provide objective information about the available range of long term care, community and institutional resources; and ensure that the individual and family understands the options, can make informed choices and determine the next steps to take. Counseling services can be provided in one session or over a series of meetings.

Options Counselors are available by phone, or by an in-person by appointment at your hospital room, rehabilitation or nursing facility. or at your home.
